The Honeoye Falls-Lima School District has four school buildings

  • Lima Primary School: Kindergarten - 1st Grade
  • Manor Intermediate School: 2nd - 5th Grade
  • Honeoye Falls-Lima Middle School: 6th - 8th Grade
  • Honeoye Falls-Lima High School: 9th - 12th Grade

Students attending HF-L must be residents of the District. We do not admit non-resident students.

    If you are unable able to print out the forms or prefer to schedule a meeting, please contact the District Registrar at (585) 624-7123.

     

  • A document noting the child's date of birth. This may include:

    • Birth certificate
    • Passport
    • Record of baptism
    • Consulate identification card
    • Hospital or health records
    • Military dependent identification card
    • Court orders or other court-issued document
    • Native American tribal document
    • Records from non-profit international aid agencies
    • Information regarding the parental relation to the student. This may include an affidavit indicating the parent(s) with whom the child lawfully resides; or that they are the person(s) in parental relation to the child, over whom they have total and permanent custody and control. The District may accept other proof, such as documentation that the child resides with a sponsor based on placement by a federal agency.
    • Two forms of Verification of Residency (any two of the following):
      • A deed or other proof of real property ownership
      • A lease or rental agreement
      • A utility bill in the parent/guardian’s name which shows an address within the district
      • The address on the parent/guardian’s driver’s license
      • The address on the parent/guardian’s voter registration
      • A recent income tax return showing the parent/guardian’s name and address within the district
      • Documentation illustrating the parent/guardian is receiving public assistance benefits at an address within the district
      • Pay stub
      • Membership cards based on residency.
    • Proof of immunization
    • A physical exam completed within the past 12 months and signed by a physician
    • The following forms, completed:
      • Pupil Registration Form
      • Request for Records
      • Residency Questionnaire
      • Student Racial and Ethnic ID Form
      • HIPPA Form
      • IEP Consent Form (if applicable)
    • Additional forms as needed

    We appreciate your patience as you navigate the registration process. The time between registration and enrollment varies. It is our desire to have your child in school as soon as possible.
